What is an Incident?

An incident is any unplanned event that interrupts normal business activities or results in harm, damage, or injury. These can arise in any workplace or operational setting and require prompt action to minimise risk and prevent escalation.

Every incident, regardless of severity, provides valuable insight and an opportunity for organisations to strengthen controls, improve processes, and enhance overall safety.

What is a Near Miss?

A near miss occurs when an incident is narrowly avoided – a situation that could have resulted in harm or damage but did not due to luck, vigilance, or quick action. While a near miss may not cause injury or damage, it provides a crucial warning to prevent future accidents from occurring.

Near misses are just as important as incidents because they often signal a gap in safety systems that could lead to more serious outcomes in the future.

Why Are Incidents and Near Misses Important?

Monitoring and reporting both incidents and near misses enables organisations to:

Reporting an Incident or Near Miss

Our simple and easy‑to‑use Incident and Near Miss solution provides a cost‑effective way for organisations to manage safety events across the entire business.

The app enables employees to record incidents and near misses quickly and clearly, helping organisations identify potential risks before they escalate into more serious issues. Every report matters, no matter how minor it may seem, as it could play a vital role in preventing a more significant incident in the future.

Once an incident or near miss has been entered, an email is automatically sent to the relevant manager so they can review the information and ensure all details have been captured. After the manager approves the entry, the record is set to “Investigation,” which is typically carried out by the H&S Team before the record is completed. Additional information, such as photos, can also be uploaded.

Graphical Analysis

A selection of graphs is provided with the system. The graphs will enable you to understand how many Incidents/Near Miss you have and also the common issues why they occurred.

This will help you pit resources in the correct areas to stop then turning into accidents.

We also use PowerBi to produce the graphs from our API. If you are using Powerbi then you can produce your own graphs.
